ACKNOWLEDGMENT OF RISK AND WAIVER OF LIABILITY

I, the undersigned, understand and acknowledge that participation in activities at Drifton Park Softball and Baseball Complex (the "Complex") involves inherent risks, including but not limited to, the risk of serious injury, permanent disability, or death. These risks may result from my own actions, the actions or inactions of others, the conditions of the premises, or the nature of the activities themselves.

I hereby voluntarily agree to assume all such risks and accept full responsibility for any injury, loss, or damage suffered as a result of my participation in any event, activity, or program held at the Complex.

I further agree, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, to release, hold harmless, and indemnify Drifton Park, its owners, employees, volunteers, agents, affiliates, and sponsors from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur while participating in activities at the Complex.

I certify that I am in good health and have no known conditions or impairments that would prevent me from safely participating in activities at the Complex.

PHOTO RELEASE

I also grant permission for Drifton Park to use photographs or video taken of me or my child during activities for promotional and marketing purposes without compensation.
